Consent to Personal Data Processing

Hereby, voluntarily, knowingly and consciously, in accordance with Article 9 of Federal Law No. 152-FZ of July 27, 2006 "On Personal Data", I give (express) my consent to the Company NOVACREWING LTD to process my personal data.

The personal data for which I give consent includes, but is not limited to:

• last name, first name and patronymic, date and place of birth, citizenship;
• data contained in qualification documents (certificates of competency, diploma endorsements, professional fitness certificates, tanker (oil, gas and chemical) service endorsements, cook qualification certificates, qualification examination protocols);
• information contained in identity documents, including passports;
• information about education, profession, skill level, professional experience;
• address of registration / residence, email address and telephone numbers;
• last name, first name and patronymic, date and place of birth, citizenship, address of registration / residence, email address and telephone numbers of my next of kin (for insurance purposes);
• bank account details for salary transfer.

The processing of personal data with my consent is carried out on the basis of documents provided either by me personally or by my authorized representative.
This statement of consent is issued without limitation.
